Moment gets stuck when dialing out

I have had the phone for about 2 weeks now and this has happened twice in the last couple of days.

I try to call someone and everything seems normal but the call never connects.  After 10 minutes it eventually disconnects.

Beyond that I am unable to end the call or do anything related to the "phone" portion of my moment.

To make things worse I am competely unable to power off my phone when this happening.  Holding down the "end-call" button does nothing  ( should let me go into airplane mode or something )

1) anyone see this before?
2) any suggestions on how to reboot when this occurs?
3) any ideas on what is occuring?

Any idea what might cause it? Or is it pretty much just random?
